The + Care card identifies the usual caregivers of patients with Alzheimer's and other dementias and regular caregivers of patients with severe disabilities.
The usual caregiver is the person who takes responsibility for the care permanently, and is / are the main reference and support of the person who cares and lives with her or spends more than 10 hours a day at home.
The Care card aims to establish positive discrimination measures to improve care and accessibility to the health system, of people with great disability and those affected by Alzheimer's and other Dementias, and the people who care for them, understanding the special situation in which find, facilitating intimacy and personalization.
